Due to the increasing number of same day cancellations and no call/no show events HAV Skincare & Wellness Center, LLC will be requiring a credit card be placed on file for all clients.

Children/Guests:

One of the most important safety concerns is about children (and other friends and family) accompanying you to an appointment. We respectfully request that you refrain from bringing children with you for your treatments. Children are not permitted in most of our treatment rooms for safety reasons and at this time we are not able to offer childcare services.

Late Arrivals:
We will do our best to accommodate you with an abbreviated service. As a courtesy to all of our clients, treatments cannot be delayed due to late arrival and will end at the scheduled time so that the next client is not delayed. We will not discount the service if you do not receive the entire scheduled time due to late arrival. Continued late arrivals will require a 25% nonrefundable deposit to book future appointments.

Cancellations:
The time needed for the service(s) that you schedule is reserved exclusively for you. If you need to cancel an appointment or reschedule one, we require 24 Hours notice for individual services and 48 hours on packages of two services or more. As we are unable to schedule another client during the time reserved for you, we will deduct the treatment time from pre-purchased treatment packages, charge a 30% of service fee to your credit card or redeem a Groupon/Livingsocial Voucher as forfeited. If a medical condition (i.e. pregnancy, cancer diagnosis) prevents you from having a service completed we will waive the cancellation fee penalty. 

Health, Allergies, or Pregnancy Considerations:
When visiting or calling for your appointment, please make sure to notify our staff if you have any health conditions, limitations, allergies to medications or fruits, physical ailments, medications you are currently taking, or if you are pregnant. We also encourage for you to alert your provider immediately of any discomfort during your treatment.

Insurance:

We do not participate with any insurance plans. If you require an itemized receipt to submit for FSA/HSA or out of network benefits please let us know. 

Retail Sales:

All supplements and skin care products are all sales final. Due to safety and tampering concerns no refunds allowed even on non-opened merchandise. 

Clinic Forms:

All clinic forms must be completed in full before arriving for appointment. They are sent electronically for your review. If you do not have access to a computer or printer to fill out our required forms please notify us ahead of time so these documents can be printed for you. Arrive 10 minutes prior to appointment to complete. Our forms are not voluntary and omitted information could have a detrimental outcome. We reserve the right to deny anyone service. If a medical condition (i.e. pregnancy, cancer diagnosis) prevents you from having a service completed we will waive the cancellation fee penalty.
